Occurrence reports are an important part of
gliding operations. The following
information from the GFA website explains
why we are all encouraged to develop and
maintain a positive reporting culture ..

Under our SMS, reporting is not limited
to activity directly related to flying
operations but includes all activities
undertaken by members. While of necessity
the Occurrence Reporting process is
developed around flying operations and
airworthiness issues any type of occurrence
can be included.
GFA members are encouraged to report any
occurrences related to club activities which
may have adverse safety implications. If in
doubt it is better to submit a report than
not.
The GFA along with the wider aviation
industry wishes to encourage the active
reporting of safety issues generally and has
adopted the concept of a "Just
Culture" which ensures that any formal
response to a reported occurrence will be
appropriate to the circumstances and will
not punish a genuine human error as it would
for gross negligence or willful violation.
The detailed information provided by the
reporting process will be used where
necessary and appropriate to investigate the
occurrence and identify possible causal
factors, while summary statistical
information will be used to identify and analyze
trends and assist in developing
safety based policies for the future.
All
clubs and all GFA members are urged to
report all accidents and incidents promptly
using the using the GFA’s occurrence
reporting portal at

as
and when they occur. This is always best
done whilst all details are fresh in
everyone's mind.

Those of you that have looked through your
copies of the Gliding Australia magazine
will see summaries of recent occurrences
have been included. Summaries for the past 3
years are viewable at

Our club hasn't been entirely silent with
reporting as I know of a number of
submissions by our members in recent times
for events including glider versus runway
marker cone, another glider versus a
different runway marker cone, air brakes
open on launch, runway
incursion, low
circuit height and wheel up landings
(including one by me at DDSC). So, you won't
be the first, you won't be the last and it
probably won't be an original event but make
sure you report it !
Our procedure for reporting occurrences is
to initially discuss the details with the
duty instructor and then lodge an occurrence
report either directly online using
http://www.irisasn.com/node/
add/incident
or by filling in one
of the blank report forms kept in the office
filing cabinet and the briefcase. The hard-copy
forms are to be mailed, emailed or handed to
me as soon as possible and I'll lodge the
details. I
automatically see any online submitted
reports. The
duty instructor will also notify me of the
details as soon as is practical.
